This evaluation is related to an earlier SPHERU project, First Nation's Health Development: Tools for Assessment of Health and Social Service Program Impacts on Community Wellness and Capacity, which was undertaken to address the information needs of community-based health directors and health care providers in tracking the impact of locally delivered programs on community health. The two key deliverables, a Community Health Indicators Toolkit and Program Logic Models, were distributed back to our partner First Nation and northern health organizations and have since been shared with other communities across Canada. In this project, we evaluated the effectiveness of our knowledge translation activities in both the dissemination and update of the key deliverables.
Goals / Outcomes: The evaluation identified how the Community Health Indicators Toolkit was being used by community partners and other health organizations, and determined how widely it had been distributed. Along with a report of findings, a web-based interactive map was created to show where the Tookit has been distributed and highlight how recipients have used it in their work. Participatory evaluation methodology involved our community partners to produce deliverables of benefit to them as well as others conducting research in this area.
